What is known, and what is missing

Measured on: Melanin index and modified MASI in facial melasma.

Three human studies exist and none of them can date this ingredient. The best split-face work (about 30 participants, 12 weeks, reads at weeks 4, 8 and 12) tests alpha-arbutin 5% together with kojic acid 2% against a hydroquinone triple-combination cream with no vehicle arm, so it isolates nothing. The 2025 Indian study is larger (124 women, 90 days) but single-arm and non-comparative, and pairs alpha-arbutin 2% with trihydroxybenzoic acid glucoside 10% and a daily sunscreen — with no control, part of that 16.3% melanin drop is the sunscreen. The third is the trap: a 90-patient randomised double-blind study with a real placebo arm and visits every 4 weeks does show its arbutin cream beating placebo on MASI, but the paper never states which anomer or what strength it used, and plain 'arbutin' means the beta-anomer unless it says otherwise, so borrowing that timeline for alpha-arbutin would be an assumption dressed as a measurement. Everything else at consumer strengths is supplier data. Missing specifically: a vehicle-controlled trial of alpha-arbutin alone with interim visits.

Rejected as combinations: PMID:39555866 (J Cosmet Dermatol 2025, alpha-arbutin 5% + kojic acid 2% vs triple combination cream, no vehicle arm); PMID:39943675 (J Cosmet Dermatol 2025, trihydroxybenzoic acid glucoside 10% + alpha-arbutin 2% + sunscreen, non-comparative, 124 women, 90 days). Rejected as an unidentified anomer: PMID:30980618 (J Cosmet Dermatol 2019, herbal cream vs 'arbutin cream' vs placebo, 90 patients, MASI every 4 weeks; concentration and anomer of the arbutin arm never stated).
